There are rail strikes planned for May and June, which may affect your journey to our hospital and community centres. If you or your child has an appointment booked, please do all you can to attend. Plan your journey ahead and allow extra time.

Who can access the team?

Our team of specialist children’s community nurses will see children and young people who have a diagnosis of asthma, constipation or eczema and are registered with a GP practice in Southwark or Lambeth. The team will see children between the ages of:

  • 2-15 years for asthma
  • 6 months-15 years for constipation
  • 0-15 years for eczema

How to access the team

You can ask your GP for a referral to the team or self-refer by completing a Health Check online. The Health Check asks about a child's health condition, their general wellbeing, and some family background information. It takes about five minutes to complete.

All families who complete the Health Check are provided with a Health Support Pack. The Health Support Pack contains practical information on how families can support their child to manage their health condition.
