Your appointment with the palliative care service

How do I get an appointment?

Your child will need a referral from your GP or hospital doctor.

Cancelling or changing your appointment

If you need to change or cancel your child’s appointment for any reason, please call the palliative care service on 020 7188 7188, extension 53823 or email paediatricpalliativecare@gstt.nhs.uk.

Please tell us as soon as possible so we can offer the appointment to another patient.

Before your appointment

There is no need to bring anything specific to your child’s appointment.

However, it is helpful to bring along your child’s red book (personal child health record) and a list of any medication that your child is taking.

During your appointment

At the appointment, you and your child will see a doctor or a clinical nurse specialist from Evelina London. You will also see a clinical nurse specialist or hospice representative from the local community team.

Appointment times may vary as each appointment is tailored to the child’s condition and needs.

As we are a teaching hospital, students and visitors may be observing the team. We will always ask your permission for them to observe your appointment.

After your appointment

After the appointment, we will write to the referring doctor to explain what happened at the appointment and what will happen next. We will send a copy of this letter to you, your GP and any other clinicians or healthcare professionals involved in your child’s care.

The letter will have a contact number if you have any questions.
