How to refer
Please contact the palliative care team for a referral form and then email it to gst-tr.ELCHPaedPalliativeReferrals@nhs.net.
If you would like to discuss any referrals or if you have any questions please contact us by calling 020 7188 7188 ext. 53278 / 53823.
Internal referrals can be made through the Electronic Patient Record (EPR) System. If it is urgent, please phone the team as well.
