How will we use the information you share with us

Guy’s and St Thomas’ NHS Foundation Trust needs to collect personal information about you in order to process your event registration form. Staff who are supporting Evelina is Growing (Evelina London, Guy’s and St Thomas’ NHS Foundation Trust) and Royal Brompton and Harefield NHS Foundation Trust staff will use this information to contact you with further details about the activities you register for.

If you take part, the data you provide will be kept for one year and will then be destroyed under secure conditions, unless you consent to continued contact about activities relating to Evelina London is Growing and the Royal Brompton and Harefield NHS Foundation Trust and King's Health Partners partnership (RBH-KHP). If you do not take part, it will be destroyed within three months of receipt.

In accordance with the General Data Protection Regulation and Data Protection Act 2018, the information provided on this form will only be disclosed to those who have a legitimate reason to see it. The lawfulness of processing this information under the General Data Protection Regulation and the Data Protection Act 2018 is that the processing is necessary for the performance of a contract, 6(1)(b).

The Trust would like to make you aware that you have the following rights regarding the data we are required to collect about you:

  • Make a Subject Access Request
  • Rectification of any incorrect information we hold about you
  • Restriction of further processing data
  • Right to be forgotten where information has been provided with consent
