Who is the service for?
Children and young people (aged 0-16) who are registered with a GP in Lambeth or Southwark and who have been treated by a doctor at Evelina London Children’s Hospital, King’s College Hospital or their GP.
How to refer to this service
All referrals must be discussed with our on call nurse.
Doctors from Evelina London Children’s Hospital, King’s College Hospital should complete a referral form via EPR. GPs can make a referral over the phone.
