Continuing care

Some children and young people have long-term health needs arising from disability, accident, or illness

The continuing care team provide care packages and support post-discharge including signposting, respite, equipment and advice.

Who can use access the service

Children and Young People (0-18 years) living in and registered with a GP in Lambeth and Southwark who meet the eligibility criteria.

How to make a referral

Any child or young person can be referred to the service by a healthcare professional providing they meet the eligibility criteria. Please note families or carers cannot self-refer.

Children and young people are assessed using the National Framework for Children and Young People’s Continuing Care. If you would like more information on the eligibility criteria please contact the team.  

Following a referral, you will receive a phone call from a member of our team. The team may then arrange a face to face assessment to discuss your child or young person’s needs.

Following an assessment, commissioners will decide how the continuing care package will be provided based on your child or young person’s needs. This continuing care package may involve other care providers, as well as educational support or support from social care.

In certain circumstances, children or young people who meet the eligibility criteria for continuing care may be provided with a Personal Health Budget. It is the responsibility of the Clinical Commissioning Group (CCG) to arrange for the Personal Health Budget.
