Your looked after children service appointment

How do I get an appointment?

We see children for initial health assessments with a doctor when they first become looked after. We carry out review assessments every six months or year depending on age. Review assessments take place with either a doctor or nurse.

Changing or cancelling your appointment

If you need to change or cancel the appointment, please contact us by calling the phone number provided on the appointment letter.

What to bring to your appointment

Please bring any forms that are requested on the appointment letter, for example Strengths and Difficulties Questionnaires (SDQs) and carers reports.

If possible, please also bring health records such as personal child health records (red book), and records of previous dental, opticians and hospital appointments.

During your appointment

At an initial health check, a doctor will ask about general health, and any worries or concerns you might have. They will ask about the last dentist visit and eye test and will also ask about school.

They will check height, weight, eyes and ears and may also listen to the child's heart.

At follow up health checks, either a doctor or nurse will carry out the appointment.
