How to make a referral
Our service is for children and young people who are:
- attending school in Lambeth or Southwark (except private or independent schools and some alternative provisions)
- not in education in Lambeth but living in Lambth, for example, home schooled or out of education
- aged 5 to 19

Please complete the Evelina London school nursing referral form for professionals, which includes information about our referral criteria.
To check on the progress of your referral you can contact the school nursing access point on 020 3049 4777 from Monday to Friday, 9am to 5pm except Bank Holidays.
