The Lancaster Model

We're using The Lancaster Model to provide electronic health questionaires to children, young people and their families in schools in Lambeth and Southwark.

By completing the questionnaire, you'll help us plan services for the future for all children and help your child get the health and wellbeing support they may need earlier on in their school journey.

We'll offer you a questionnaire to complete when your child is in:

  • Reception
  • Year 6
  • Year 9

Reception

Starting school is an exciting time for children and their families, but it can be daunting. In order to tailor our support, we're offering all parents and carers of children in their reception year at a school in Lambeth and Southwark the opportunity to complete a health and wellbeing questionnaire online.

The questionnaire will help to identify any physical, developmental, or emotional needs for your child that may be worrying you. This means that plans and support can be put in place early and makes sure children have the best possible support in their first school year.

You'll receive a letter from your school when it's time for you to complete the questionnaire for your child.

Year 6

Transition from primary to secondary school is a big milestone to navigate. We'll be offering children in Year 6 the opportunity to complete a questionnaire at school and they may be offered a health assessment based on their results. Parents and carers will receive a letter when their child's school questionnaire is going to be offered.

Year 9

We're passionate about enabling young people to tell us how they feel about their health, what worries them the most and what they want to know more about as they navigate into adulthood. They'll be offered a questionnaire to complete at school that will give them the opportunity to do this.
