Who can refer
Children can be referred by any healthcare professional. Referrals should only be made with the informed consent of children's parents or carers. When a referral is made by anyone other than the child's GP, their GP should be advised of the referral.
We provide routine and specialist care across all South East London boroughs and specialist care for the borough of Croydon.
How to refer
Please send the audiology referral form (Word 201KB) by email to gstt.cypacreferrals@nhs.net.
EPIC users can place an internal referral using the REF461 order form.
